Fatty Liver and PCOS: How to Reverse It With Nutrition and Lifestyle

Here's the good news and the not-so-good news about fatty liver: it's one of the most common conditions linked to PMOS (PCOS), and also one of the most reversible, if you catch it early.

A national estimate using the Canadian Health Measures Survey (CHMS) from 2009-2019 suggests between 1 in 3 and 1 in 2 adults have fatty liver. People with PMOS (PCOS) are up to four times more likely to develop fatty liver than those without. 

While these stats can be overwhelming, there is a silver lining: small, consistent changes can meaningfully shift the trajectory of this condition.

Here's what fatty liver actually is, why it's so closely linked to PMOS (PCOS), and three evidence-backed nutrition strategies you can start using today.

What Is Fatty Liver Disease?

Fatty liver, more specifically non-alcoholic fatty liver disease, happens when excess fat builds up in the liver, exceeding 5% of its total weight, unrelated to alcohol consumption. Risk factors include having insulin resistance, sarcopenia, age, nutritional factors, sleep duration, metabolic syndrome and more.  

Fatty liver is often called a silent disease because early stages tend to have minimal symptoms, or symptoms that get chalked up to stress or fatigue. That's why early detection matters. If you're in a higher-risk group, such as people with PMOS (PCOS), diabetes, or insulin resistance, your doctor will typically monitor liver enzymes through blood work and may order an ultrasound.

The Link Between PMOS (PCOS) and Fatty Liver

As we established, people with PMOS (PCOS) are up to four times more likely to develop fatty liver. While the exact mechanism isn't fully understood, two hallmark features of  PMOS (PCOS) appear to play a key role:

  • Hyperandrogenism: higher levels of male sex hormones like testosterone, which may be linked to an increase liver fat accumulation.

  • Insulin resistance: a common underlying feature of  PMOS (PCOS)  where cells don't respond properly to insulin. Because insulin is a growth hormone, insulin resistance can drive more fat storage in organs, including the liver.

Over time, this fat buildup can lead to liver inflammation and damage, which is often reflected in elevated liver enzymes and triglycerides on blood work. 

Tip 1 to Reverse Fatty Liver: Limit Excess Fructose, Especially From Beverages

Fructose is a sugar that occurs naturally in nutritious foods like whole fruit, and it's also widely added to processed foods and beverages, like soft drinks, fruit juice, and many packaged snacks, often in the form of high-fructose corn syrup or table sugar (which is half fructose).

Whole fruit itself is an excellent source of antioxidants that support liver function, and typically not something to be “worried” about. However, with fatty liver, you may want to pay attention to other sources of fructose and its concentration.

When large amounts of fructose reach the liver, it converts the excess into fat through a process called lipogenesis. Research shows the same amount of fructose is far more likely to contribute to fatty liver when it comes from beverages, like fruit juice or soft drinks, than from whole fruit. That's because whole fruit comes packaged with fibre, which slows sugar absorption, along with antioxidants that actually support liver health.

Concentration matters too. A large, concentrated dose of fructose, like a glass of juice, hits your bloodstream and your liver much faster than the same amount spread out across the day in whole food form. 

A trial found that just six weeks of reducing (not eliminating) dietary fructose is linked to a small, but still significant reduction in liver fat.

Practical swaps: choose whole fruit over fruit juice, enjoy soft drinks less often, and pair dried fruit with nuts or seeds to slow absorption of fructose.

Tip 2: Increase Antioxidant-Rich Foods, Especially Vitamin E

Vitamin E is a potent antioxidant, and research has found a link between higher intake of vitamin E-rich foods and reduced fat buildup in the liver. Good sources include almonds, other nuts and seeds, spinach, pumpkin, and mango. Try adding almond butter to a smoothie or oatmeal, cooked leafy greens to a stir-fry, or fresh mango as a snack.

Vitamin E supplements are a bit more mixed.. A recent review found that vitamin E supplements helped improve liver enzyme levels  and reduced liver scarring and fat buildup in people with fatty liver. The biggest improvement in scarring showed up at doses above 600 IU/day, or after taking it for 12+ months.

Older studies (from 2005) had linked doses above 400 IU/day to a higher risk of dying from any cause. Though, this is something that’s not reflected in newer research. In fact, a newer, larger review of 57 trials conclude that vitamin E supplementation appears to have no effect on all-cause mortality at doses up to 5500 IU/day. 

Given the conflicting evidence, high-dose vitamin E supplementation isn't something we typically recommend, particularly for people on blood thinners or statins, or those with diabetes. If you're considering a supplement, talk to your dietitian or doctor first to make sure it's appropriate and screen for contraindications.

Tip 3: Adopt a Plant-Centric Eating Pattern

Research links a plant-centric eating pattern to reduced liver fat and improved insulin sensitivity, both directly relevant to PMOS (PCOS) and fatty liver management.

This doesn't mean going fully vegan. Rather than focusing on what to eliminate, think in terms of abundance: what plant foods can you add to what you already enjoy eating? This includes simply adding more variety of whole grains, legumes, nuts, seeds, and produce, alongside moderate amounts of fish, dairy and eggs.

The underlying concept, more plants, more often, can be adapted to any cuisine. For example, a Chinese Canadian version might include tofu, bok choy and kailan, persimmons with cashews, and red bean soup for dessert.

Bonus Tip: Move Your Body

Exercise supports liver health with or without weight loss, by lowering triglycerides and inflammation and improving insulin sensitivity and liver fat metabolism. This matters because weight loss alone, often the only advice people hear from a doctor, isn't the full picture and doesn't offer tangible, evidence-based behavior support.

Aim for 150 to 200 minutes per week of a mix of aerobic and resistance training, in whatever form you actually enjoy. It doesn't have to mean the gym. Swimming, cycling to work, dance class, yoga, and martial arts all count. Movement you enjoy is movement you'll stick with.
