Cardio with PMOS (PCOS): Is It Harmful? Plus, 5 Things to Eat Before a Workout
If you've spent any time in PMOS (formerly PCOS) spaces online, you've probably seen conflicting advice about exercise. One video says cardio spikes your cortisol and makes things worse. The next says you need to move more. A third insists you have to "cycle sync" your workouts or you're doing it wrong. No wonder exercise feels as confusing as food sometimes!
In this post, I'll cover what the research actually says about exercise and PMOS (PCOS), bust a couple of common myths and share five pre-workout snacks to help you fuel up and recover well.
Why Exercise Feels So Confusing with PMOS
I tend to see two patterns in the people I work with.
The first is people who care so much about doing it "right" that they spend more time researching the perfect workout than actually moving. Exercise ends up feeling intimidating instead of enjoyable (kind of like what happens with food when you pile on too many rules).
The second looks almost opposite. I’m speaking to those of you who are high performers and super consistent with exercise, who pushes through a fasted workout on black coffee before a packed day. Rest days bring guilt. Over time, training hard without enough fuel or recovery leads to burnout, and it's common to wonder why symptoms are flaring despite "doing everything right."
Different as they look, both come from the same place: uncertainty about what exercise is supposed to look like with PMOS (PCOS), and a relationship with movement that isn't quite sustainable (yet).
How Exercise Actually Helps With PMOS (PCOS)
Here's the good news: physical activity is one of the most well-supported tools we have for managing PMOS, no matter which type of movement you choose.
Exercise reduces your blood sugar
Insulin resistance affects roughly 80% of people with PMOS. When insulin isn't working efficiently, blood sugar stays elevated longer than it should.
Here's the fun part: whenever you exercise, your muscles act like a sponge. Your muscles soak up sugar straight from your bloodstream, independent of insulin. And that effect doesn't stop when your workout ends: your insulin sensitivity can stay elevated for up to 24 hours after you exercise.
Exercise protects your heart
PMOS isn't only a reproductive condition. It also comes with a higher long-term risk of high blood pressure, high cholesterol and type 2 diabetes. That's exactly why heart health screening matters as part of preventative care, not just menstrual-related symptom management. Exercise is one of the most accessible tools you have in your toolbox to protect this organ, lifelong.
Exercise supports your mental health
A review of exercise studies in PMOS found every single one showed improved quality of life, and about half also showed real improvements in depression and anxiety.
There's no single "best" workout
This one's worth sitting with, since so much online content implies otherwise. Research comparing HIIT, moderate cardio and resistance training found no meaningful differences in outcomes between them. Walking, lifting, cycling, swimming, running, pilates, yoga, they're all reasonable choices. What matters most is picking something you'll actually enjoy and stick with, because consistency is what drives the benefit.
Myth: "Cardio Is Bad Because It Spikes Cortisol"
This is one of the most common claims I hear, so let's clear it up.
Cortisol is a stress hormone, and yes, exercise raises it. But that's not a harmful thing, inherently. It's cortisol doing its job: releasing stored energy so your muscles have fuel to work with! That rise is supposed to be temporary. With good fueling and recovery, it comes back down afterward. In fact, research suggests that exercise-induced cortisol response may actually help prepare you for subsequent stressors. Meaning, exercise can help you build stress resilience.
If you do feel like you’re “burning out” after exercising however, the problem may be that your body doesn’t have what it needs to recover. Ask yourself:
Am I fueling well before and after I move?
Am I getting enough sleep?
Am I building on rest days?
Is my overall stress load manageable?
A better question than "is cardio bad for my cortisol" is: "Am I supporting my body to recover from the stress load of the day?"
Myth: "You Have to Cycle Sync Your Workouts"
Cycle syncing (aka following a strict workout plan matched to each phase of your cycle) is trendy online, but I don't recommend a rigid template pulled from social media.
That said, there's a kernel of truth in it. Your hormones do shift across your cycle, and that can genuinely affect your energy and capacity. If you feel like doing something gentler during your luteal phase, that's a totally reasonable response to how you feel. That said, you might actually feel totally capable of doing your usual workout routine. Lean into how your body is feeling.
The key difference is between a rule ("I can't do cardio because I'm in my luteal phase") and a check-in ("What's my capacity today, and what feels realistic?"). The second lets you combine the research with what your body is actually telling you, instead of outsourcing the decision to a template.
5 Pre-Workout Snacks to Fuel and Recover Well
The simple formula: quick carbs + a bit of protein, about 1-2 hours before you move.
Banana or plantain + peanut butter or tahini: fast carbs plus protein, easy and portable.
Greek yogurt + berries: protein from the yogurt, easy carbs from the fruit. Add honey or granola if you like.
A smoothie: endlessly flexible. Mango and yogurt, strawberry-matcha with soy milk, or mixed berries with nut butter all work.
Oatmeal or millet porridge: great fuel, and you can prep it ahead as overnight oats.
Rice cakes + a protein source: rice cakes for quick energy, paired with eggs, tofu, or cottage cheese if you have time, or tahini if you're short on it.
The Bottom Line
Exercise with PMOS isn't something to fear or overthink. It's one of the most valuable tools we have for blood sugar, insulin sensitivity, heart health, and mental wellbeing, and there's no single "correct" workout required to get those benefits.
What often gets missed is that exercise doesn't happen in isolation. How you fuel it and recover from it matters just as much as the workout itself. A better question than "what's the perfect exercise for PMOS" is: "how can I support my body before, during, and after I move?" That usually comes back to the basics: eating enough, sleeping well, and choosing movement you can actually sustain.
